I do see the mods present in game after the download status stops but this is only possible if you restart the game. No this is false there is no anti inject for bo3, simply the newest patch/update broke all MXT's before 1.08 (current version is 1.09) you can find the new MXT in the official MXT discord. Btw Process Hacker 2.0 still works with bo3. in all my searches i haven't found MXT Mod Menu 1.09. I thought the newest one was MXT_MENU_1.0.5.
Custom zombie maps are installed through the Steam Workshop. The only way you can get this to work is to own the game on Steam, download the custom maps, then move them from the steam workshop folder to your bo3 mods folder once they are downloaded.
